Report Your Annual Progress

You will need Adobe Acrobat Reader, available as a free download, to view some of the files on this page.

Facility Partners are required to report on their progress each year. The Annual Reporting Form for FY2012 is due January 31, 2013.

New Facility Partners: If you joined prior to October 1, 2012, please report your annual progress for fiscal year 2012 by January 31, 2013.

Existing Facility Partners: Please report your annual progress for fiscal year 2012 by January 31, 2013.

FEC Facility Partners may submit an Annual Reporting Form to the FEC:

  1. Login to the FEC reporting system at FedCenter.
  2. Select "Baseline Survey & Annual Reporting" from the FEC Reporting Menu in the left sidebar.
  3. In the Facility Partner - Annual Reports table, look for the row with "FY2012" in the Fiscal Year column and "Annual Report" in the Report Type column.
  4. Click on your Facility name in that row to enter, update or submit your Annual Reporting data.

An Adobe Acrobat file of the FEC Annual Reporting Form (PDF, 193 KB) and a Microsoft Word file of the FEC Annual Reporting Form (DOC, 318 KB) are available to help you collect the necessary information. Completed Annual Reporting Forms must be submitted online through FedCenter.
